HealthCore: Digital Assessments through the Continuum of Care

The integration of digital assessments in clinical trials is challenging at scale due to the lack of a unified data infrastructure. Research teams currently develop and implement island solutions for specific studies, that are not scalable and fragment the data landscape. Compounding this, valuable data from clinical workflows are not effectively utilized, limiting the ability of AI-research to effectively develop and train models in the rehabilitation space. There remains a pressing societal need to optimise rehabilitation in terms of delivery efficacy, cost efficacy, and outcome prediction.

We propose a platform solution for end-to-end support of digital assessments, aiming to create a disease-independent framework for evaluating medical interventions. Our vision involves gathering diverse patient data from various sources. HealthCore is the framework designed for streamlined data collection across multiple clinics in Switzerland.

The system is designed as a hub and spoke system, where each clinic in our network acts as a spoke, referred to as a clinical node. Every node serves as an entry point for data, which is then securely packaged, encrypted for transmission, and sent to our hub. The hub is comprised of an on-premise open-source solution, and could in theory be replicated in similiar fasion for further hubs. Upon arrival, data is decrypted and stored in our object storages. An event broker automatically notifies our analytics services that data has arrived for further processing. After processing, we feed data back into clinical routine through dashboards. The event broker informs the visualization service of new data for processing, and a data integration tool then retrieves the processed output data, making it available for our visualization tool, which is accessible to researchers and clinicians. Through this platform, assessment deployment is facilitated, visualization is automated, and researchers can focus on prototyping.

The ambition of the HealthCore project is to grow into a national lighthouse project by establishing a secure and scalable data platform that supports trustworthy healthcare AI solutions from development to deployment. HealthCore aims to enable clinical data collection at unprecedented levels of detail to inform personalized, real-time AI prediction and treatment models. At the same time, HealthCore is lightweight, unobtrusive, and simplifies the currently tedious clinical evidence generation process.
